You will be asked to fill out the following forms prior to your first visit with us. For your convenience, you may open and print these forms from this site and fill them out ahead of time. Be sure to bring the completed initial questionnaire, registration form, and consent form with you. The privacy notice is included here for your records, but you do not need to bring it with you. For your first appointment, you will also need to bring your insurance card(s), one form of identification, and any written prescription or referral your doctor may have given you.
